There may be variety of HTML code format to place favicon.ico on the webpage depends on what web designer intend the picture to look like. However, these codes should be based on the following favicon HTML code:

Your favicon.ico should be saved in the root directory and the size should be 16px x 16px.

You can use .ico format or if you want your favicon to be animated you can use .gif format, but be aware that your .gif favicon wont work with some minor web browsers.
